In case of emergencies, please call 911 or contact a local Poison Control Center (1-800-282-3171 - available 24/7).

Q: How do I discontinue a product brand registration?
A: To
discontinue a product brand registration during the annual renewal period:
1) Write D1 and initial next to each product brand to start/denote the first
(1st) year discontinuance.
2) Write D2 and initial next to each product brand to denote the second (2nd)
year discontinuance.
3) Line-through and initial next to each product brand that is not being
renewed or is being cancelled.
